3. Certificate Regarding Non-Black Belt waiver Participants (if Applicable) – Click Here to Download
4. Registration Fee - $90 for pre-registration, $115 after September 16th, 2010
5. Proof of US Citizenship - Passport or Birth Certificate
6. A copy of your United States Judo Inc Membership Card – Click Here for USJI Membership Application form.


If Non-US Citizen
